Hazard Identification and Scaffolding risk assessment

The following Risk Assessment for scaffolding access to work at height and under the scaffolding to identify the hazard, risk harm, control, and prevention. Measure the level of risk during the work on the scaffolding and following the safety rules and identifying the low, medium and high risk at work.

Hazard Identification

  1. Incorrect or incomplete erection of scaffolding.
  2. Use of unsecured access ladder.
  3. The collapse of scaffolding from overloading, inappropriate scaffolding material or unstable footing/base.
  4. Inadequate working platform.
  5. People are falling from the height.

Risk harm identification (Why, who, When, What, How, and Where )

  1. The scaffold can collapse and fatality/serious injury to the operatives working on the platform.
  2. Scaffold collapsing injuring operatives on the scaffolding and operatives at or near the scaffolding.
  3. The Measure of access ladders causing serious injury to operatives.
  4. The risk of the collapse of access ladder.
  5. Fatality/major injury to the men working at height.

Control measure and prevention

Controlling the risk with discipline, PPE, Control, Isolate, Reduce and Eliminate

  1. Scaffolds must only be erected, dismantled or altered by competent personnel under supervision. 
  2. Follow scaffolding tag system.
  3. All scaffold and working platforms must be inspected by a qualified scaffold Engineer/ Supervisor before use and weekly inspection every seven days after that.
  4. All inspection reports must be recorded in the scaffold register for reference and follow up on comments.
  5. Appropriate ladders should be used to climb the scaffolding and should be lashed/clamped to the structure.
  6. Scaffolding should not be overloaded and must be kept clear for access along the scaffold.
  7. Safety harness must be worn and secured at all time to the life lines or an approved anchorage point which can withstand the load of the people who anchor.
  8. Guardrails and toeboards are required to all working platforms of the height of two (2) meters and above.

Controlling the risk with discipline, PPE, Control, Isolate, Reduce and Eliminate

  1. Provide toe-boards on the scaffold.
  2. Provide adequate platforms and adequately clamped.
  3. Provide barriers and warning signs below the scaffolding.
  4. As far as possible do not allow anybody to work under the scaffold platform
